The only thing that did happen was toward the end when my hand was getting tired and my palms were a little sweaty, I didn't rack the slide back far enough because my hand slipped and the round got stuck with the slide open. This happened twice and I attributed both times to not pulling the slide back far enough. I don't consider this a design flaw, I consider it user error. Overall I am very happy with the gun.

I'm hoping you can get magazines without the extension.
I know some people like those but I'm in the camp that I get a small gun for a reason and I don't want to increase its over-all size with a magazine extension.
Or maybe those come off?
I have to admit I haven't read or listened to all of the reviews.
